Release Forms

There are two release forms that must be signed before you participate in the study.

The first releases the study coordinator from any liabilty associated with this site. The second is used to grant permission to the study coordinator to publish the participant's personal information. (The participant decides which pieces of information to have published.)

Here are the instructions to complete both forms. If you have trouble accessing or printing the forms, contact the study coordinator.

1. Display the release of liabilty, then print it.

2. Display the information release form, then print it as well.

3. Read both forms carefully.

4. Complete both forms and sign where indicated. (Note that both forms ask for your FTNDA kit number, which you won't have until you've ordered your kit from FTDNA.)

5. If someone other than the participant is the contact, the contact person will need to complete both forms as well.

6. Mail the signed forms to the address found at the bottom of each form.
